It's great that you are committed to exercising in spite of your condition. Before starting a new program you should consult with your doctor, especially to address your medical issues.
Start out small with the exercising and gradually progress. You can brake up your sessions into 5-10 minute intervals for now- one in the morning and one later in the day if that helps. Your body will basically respond the same way it would to one longer session.
In order to minimize joint pain and swelling, try aquatic exercises, which are probably the best form of exercise you could do in your current situation. The water will support your body and offers resistance with minimal to no pressure on the joints and ligaments. You may also try other similar exercises that are easy on the joints and promote flexibility like Yoga and pilates. You could do Yoga or Pilates tapes right at home. If you have access to a gym, you may want to consult with a trainer to guide you and get you started on a minimal strength training and stretching routine that is safe and effective for your condition.
